The Partnership’s multi-sector stakeholders drive the collaborative’s activities to develop resources and educational opportunities to advance health and social care alignment. The Partnership serves as a unique and neutral forum for representatives and leaders from across health and social sectors to share their individual expertise. All Partnership endeavors elevate the critical role of the community and community-driven services and supports to address social determinants of health (SDOH) and promote health equity.
