June Simmons, MSW

President & CEO, Partners in Care Foundation
Partnership Co-Chair

June is a nationally recognized leader in healthcare administration. Since founding Partners in Care in 1997, she has spearheaded the development and scaling of evidence-based care models, particularly for managing chronic conditions and improving care coordination. Simmons is a frequent speaker and has received numerous awards for her contributions, including induction into the California Social Work Hall of Distinction in 2018. She has co-founded several key initiatives, including the Evidence-Based Leadership Council and the Los Angeles Alliance for Community Health and Aging. June also serves on the board of NCOA and the EBLC, the Steering Committee for the USAging Business Institute, and leads multiple collaboratives. She is a member of the advisory committee for the NCQA accreditation of HCBS/LTSS.

Timothy McNeill, RN, MPH

CEO, Freedman’s Health Consulting
Partnership Co-Chair

Timothy McNeill, NP has 30 years of professional experience in healthcare, first in the United States Navy and then in hospitals and healthcare centers. As founder and CEO of Freedmen’s Health Consulting (FHC), McNeill leads the firm’s efforts specializing in the implementation of innovative models of care and serving groups including the U.S. Department of Health and Human Services (HHS), U.S. Administration on Aging (AoA)/Administration for Community Living (ACL), and various foundations and national non-profit organizations. Under HHS/ACL contract, McNeill is the lead TA provider to establish and support integrated networks to deliver new models of care that address medical risks and social determinants of health supporting value-based contracting in twenty-six states.

Autumn Campbell

Director, Partnership to Align Social Care

Autumn Campbell is the Director of the Partnership to Align Social Care (Partnership) and Principal at DueWest Strategies. At the Partnership, Autumn leads the collaborative’s efforts to align leaders among community-based organizations, health plans and systems, national associations and advocacy organizations, philanthropic organizations, foundation, academic institutions, federal agencies, and other key participants to co-design strategies to enable successful partnerships between health care organizations and networks of community-based organizations (CBOs) delivering social care services. Autumn has a broad portfolio of experience in the policy and advocacy arena and has worked for nearly 20 years for DC-based associations and nonprofits including serving as the Senior Policy Director at USAging.

Jeremiah Silguero, MPH

Senior Manager, Partnership to Align Social Care

Jeremiah Silguero is a passionate and driven public health professional with research experience and an interest in health policy and law. Prior to coming on board with the Partnership to Align Social Care, he was a policy and research associate at the National Alliance for Hispanic Health. During his time at the Alliance, he worked closely with community-based organizations on COVID-19 vaccine projects helping to bring much needed COVID-19 resources and vaccines to underserved Hispanic communities through promotora networks. He also supported the Alliance’s press release and media communications and helped produce policy and research content for the organization. He holds a Master of Public Health degree from Boston University’s School of Public Health and currently resides in Washington, D.C.
